• Communicates with the physician regarding the patient’s needs and reports changes in the patient’s condition; obtains/receives and documents physicians' orders as required.

ED NURSE (RN CASE MANAGER) – Qualifications/Experience
• Current ed Nurse (RN) license from the State of Virgina. BSN preferred.
• Knowledge and understanding of the hospice philosophy and hospice criteria for ission and the ission process.
• Knowledge of private insurance, Medicare as well as State and Federal regulations.
• Ability to document in an EMR system
• Previous hospital, physician practice, LTC or ALF experience
• Maintain a current R certification
• Strong Presentation Skills
• Previous sales or service experience preferred
• Excellent oral and electronic communication skills.
• Proficient computer skills and electronic medical records required.
• Must be able to travel throughout the territory as needed.

The Speech Language Pathologist in Home Health is responsible for the assessment and evaluation of patient care needs related to treating speech and language disorders, and functional training in communication, swallowing, and cognitive impairments. Based on this assessment and evaluation, the Language Pathologist works to help determine a treatment plan, performs interventions aimed at improving and enhancing the patient's well-being, and evaluates the patient's progress.
