  • Performs routine office duties such as answering the telephone, receiving residents and filing records, reports, case histories and statistical studies, etc.

  • Makes recommendations to Physical Therapist about how methods and procedures can be improved.

  • Demonstrates and instructs residents and nursing personnel on the use of medical appliances, exercises, positioning, transferring and any other appropriate methods.

  • With input of Physical Therapist, prepares residents discharge plans in coordination with other team , including the timely ordering of all equipment and or assistive devices required.

What You Need: 

  • Must have graduated from an accredited Physical Therapy Assistant program

  • Must have a valid professional license and registration for state of practice, VA.

  • Experience with long-term care and/or geriatric care is preferred

CERTIFIED NURSING ASSISTANT (CNA) – Position Summary

The Certified Nursing Assistant (CNA) is a member of the interdisciplinary team who works under the supervision of a ed Nurse (RN) and performs various services for a patient as necessary to meet the patient’s personal needs and to promote comfort. The Certified Nursing Assistant (CNA) is responsible for observing the patient, reporting these observations to the RN and documenting observations to the RN and documenting care. 

 

CERTIFIED NURSING ASSISTANT (CNA) – Essential Duties & Responsibilities

  • Personal Care: Provide personal care to patients, including baths, back rubs, oral hygiene, shampoos, and changing bed linens as assigned.
  • Patient Assistance: Assist patients with dressing, undressing, feeding, and other daily activities as assigned.
  • Vital Signs: Take and record oral, rectal, and axillary temperatures, pulse, respiration, and blood pressure as ordered, ensuring competency in these skills.
  • Skin Care: Monitor and care for patients’ skin to prevent tissue breakdown over bony prominence.
  • Reporting: Observe and report any significant changes in the patient’s condition to the ed Nurse.
  • Mobility : Assist with patient ambulation, exercise, and use of bedpans and urinals as directed by the nursing staff.
  • Environmental Maintenance: Keep patient living areas clean and orderly and assist with light laundry needs.
  • Documentation: Adhere to the organization’s documentation procedures, ensuring accurate and timely record-keeping.
  • Professional Conduct: Maintain high standards of personal and professional conduct, respecting the confidentiality and privacy of patients, caregivers, and team .
  • Safety: Ensure patient safety by using equipment properly and adhering to safety protocols.
  • Professional Development: Participate in personal and professional growth activities, including attending at least twelve (12) hours of continuing education programs per year. Engage in quality assessment performance improvement initiatives and attend interdisciplinary team (IDT) meetings.
  • Respectful Care: Be respectful of patients' and families’ environments and personal needs, providing care that is sensitive to their preferences.

 

CERTIFIED NURSING ASSISTANT (CNA) – Qualifications/Experience

  • Currently licensed in VA as a Certified Nursing Assistant 
  • High School Diploma strongly preferred 
  • Self-directed with the ability to work with little direct supervision. Is secure with issues of death/dying. Provides a calm manner when in a patient’s home.

Responsibilities:

  • Design and execute marketing strategies to promote PathWell Health's Hospice services to targeted facilities and referral sources.
  • Forge and maintain relationships with key stakeholders in ALFs, SNFs, Community Organizations, Senior Centers, LTCFs, and medical practices, particularly those specializing in oncology, cardiology, pulmonology, and family/internal medicine with a geriatrics focus.
  • Present the benefits and value of PathWell Health’s comprehensive care services through persuasive sales presentations.
  • Engage with medical professionals to inform and educate about our unique hospice offerings.
  • Drive referral generation by identifying, qualifying, and following up on leads from various sources to maintain a strong pipeline of potential patients.

Qualifications:

ED NURSE (RN CASE MANAGER) – Position Summary

The ed Nurse (RN Case Manager) plans, organizes and directs hospice care. Experienced in nursing, with emphasis on community health education/experience. The ed Nurse (RN Case Manager) builds from the resources of the community to plan and direct services to meet the needs of the individual and families within their homes and communities.  

 

ED NURSE (RN CASE MANAGER) – Essential Duties & Responsibilities

Patient Assessment and Care Planning

Completes initial, comprehensive, and ongoing assessments of patients and family to determine hospice needs. Provides a complete physical assessment and history of current and previous illness(es).

Uses health assessment data to determine nursing diagnosis.

Assesses and evaluates patient’s status by writing and initiating a plan of care, regularly re-evaluating patient and family/caregiver needs, and participating in revising the plan of care if necessary.

Develops a care plan that establishes goals, based on nursing diagnosis and incorporates palliative nursing actions. Includes the patient and the physician in the physician’s plan of care.

Clinical Care and Nursing Interventions

Provides professional nursing care by utilizing all elements of the process.

Initiates appropriate preventative and rehabilitative nursing procedures. isters medications and treatments as prescribed by the physician’s plan of care and/or orders.

Assists the patient with the activities of daily living and facilitates the patient’s efforts toward self-sufficiency and optimal comfort care as needed.

Provides and maintains a safe environment for the patient.

Patient and Family Education and

Counsels the patient and family in meeting nursing and related needs.

Provides health care instructions to the patient as appropriate per assessment plan.

Teaches the patient and family/caregiver self-care techniques as appropriate. Provides medication, diet, and other instructions as ordered by the physician and recognizes and utilizes opportunities for health counseling with patients and families/caregivers.

Works in cooperation with the family/caregiver and hospice Interdisciplinary Group to meet the emotional needs of the patient and family/caregiver.

Documentation and Communication

Completes, maintains, and submits accurate and relevant clinical documentation regarding the patient’s condition and care given. Records pain/symptom management changes/outcomes as appropriate in the EMR at bedside.

Communicates with the physician regarding the patient’s needs and reports changes in the patient’s condition; obtains/receives and documents physicians' orders as required.

Coordinates arrangements for equipment and other necessary items and services are available.

Interdisciplinary Collaboration and Teamwork

Works in concert with the interdisciplinary group.

Attends interdisciplinary group meetings.

 

ED NURSE (RN CASE MANAGER) – Qualifications/Experience

   •   Current ed Nurse (RN) license from the State of Virgina.  BSN preferred.

   •   Knowledge and understanding of the hospice philosophy and hospice criteria for ission and the ission process.

   •   Knowledge of private insurance, Medicare as well as State and Federal regulations.

   •   Ability to document in an EMR system

   •   Previous hospital, physician practice, LTC or ALF experience

   •   Maintain a current R certification

   •   Strong Presentation Skills

   •   Previous sales or service experience preferred

   •   Excellent oral and electronic communication skills.

   •   Proficient computer skills and electronic medical records required. 

   •   Must be able to travel throughout the territory as needed.

Responsabilidades y Deberes

The Speech Language Pathologist in Home Health is responsible for the assessment and evaluation of patient care needs related to treating speech and language disorders, and functional training in communication, swallowing, and cognitive impairments. Based on this assessment and evaluation, the Language Pathologist works to help determine a treatment plan, performs interventions aimed at improving and enhancing the patient's well-being, and evaluates the patient's progress. 

ESSENTIAL DUTIES AND RESPONSIBILITIES 

The essential functions include, but are not limited to the following: 

  • Provides services within the scope of practice as defined by the state laws governing the practice of speech therapy, in accordance with the plan of care, and in coordination with other of the health care team 
  • Evaluates the patient's level of function by applying diagnostic and prognostic functional ability tests. Assists the physician in the development of the therapy plan of care 
  • Treats patients to communicate effectively by expressing thoughts according to the patient's condition using acceptable standards of practice 
  • Observes, records, and reports to the supervising nurse and/or physician the patient's response to treatment and changes in the patient's condition 
  • Instructs the patient, the family and/or caregiver, and other of the health care team in areas of speech therapy in which they can participate 
  • Visits patient according to the plan of treatment, completes a progress note for each visit, and submits progress notes per Caring Angels Home Health policy. 
  • Completes all patient evaluations and develops the SLP plan of care within state-specific guidelines. Reports outcomes of evaluation, goals, and anticipated projected frequency of care to supervising nurse within 24 hours of the evaluation. 
  • Participates in staff conferences and committees as necessary 
  • Provides in-services to agency staff upon request of supervising nurse 
  • Sends the supervising nurse and/or the physician a written summary report on the patient's condition at least every 60 days 
  • Performs other duties as assigned 

MINIMUM QUALIFICATIONS (KNOWLEDGE, SKILLS, AND ABILITIES) 

  • Master's degree in Speech Therapy or Speech-language Pathology 
  • Must be currently licensed in Speech Therapy in Virginia (required) 
  • Current R certification (preferred) 
  • Driver’s License 
  • Car Insurance 
  • Car Registration 
  • Strong leadership skills and ability to work collaboratively with a large team 
  • Strong clinical assessment, organizational, and communication skills 
  • Ability to handle multiple assignments simultaneously 
  • Ability to utilize computers and other electronic devices for tasks such as timekeeping, in-servicing, and documentation 
  • Bilingual (Spanish) preferred
